A clear distinction between the public website, working enquiry forms and Care Loop forms completed on your device.
Public browsing and private accounts
You can browse the directory and send capacity enquiries without an account. Private application and owner workflows are limited to owner-enabled email addresses with verified website accounts. Creating an account does not grant access. Pilot entry is separate from clinician approval. The owner can enable or disable invitees. Public directory records are served through an endpoint limited to approved display fields; private evidence and account records are not exposed. Profile photographs and logos remain public media assets; use only images you have permission to publish.
Directory and demonstration
The directory can contain published clinician profiles and clearly labelled test or example profiles. Test profiles do not represent approved membership. Care Loop provides discipline-specific checklists and appointment forms for physiotherapy, speech pathology, occupational therapy, dietetics and registered nursing. Entries are held in memory in the current browser tab. They are not sent to a site API, saved in browser storage or stored in a site database. Reloading or closing the tab clears them. Downloaded reports and email files remain on your device and must be managed through your approved clinical-record and communication processes.
Enquiries and applications
Submitting a form sends the information you enter to the site's Wix forms dashboard for review by the site owner. This includes your contact details and, for applications, your professional background. Submit only information needed for your enquiry or application.
Do not send patient names, clinical records or urgent requests through general enquiry or application forms. Capacity enquiries have separate fields for brief, non-identifying diagnosis and support context. General enquiries are not clinical referrals.
Five-minute questionnaire
The feedback questionnaire asks about your role, MND care experience and views on the concept. It does not ask for your name. A comment and follow-up email are optional. Responses are stored in the site owner’s Wix forms dashboard. Do not include patient information. Providing a contact email allows the site owner to follow up about your pilot interest.
Capacity and willingness enquiries
Select public clinician profiles and prepare a non-identifying capacity enquiry. Selected public profile IDs are kept in session storage for the current browsing session and appear in the enquiry URL. Unsent details remain in the open tab. Without requiring an account, Send enquiry sends your name, reply email, organisation, service area, funding, timeframe, diagnosis and its status, and optional diagnosis details, support needs and relevant precautions through Wix email to the selected contacts. Each contact receives a separate message and replies directly to your email; replies are not tracked on this website.
The website retains private send references, sender account or random visitor references, public profile IDs, payload digests and email transmission statuses for reliability and sending limits. It does not store the diagnosis or enquiry text in those records. Browser/account, reply-email and overall pilot limits help prevent repeated sends. Wix email retains email content for 30 days. Downloading an email draft does not send it; sending that draft uses your email provider. Do not enter client identifiers or clinical records. Use the clinician’s secure referral process after they confirm interest. Care Loop forms remain device-only and are not submitted through this enquiry function.
Physiotherapy entry assessment
The scored assessment collects your professional name, registration number, work-history evidence summary, declared experience and eligibility declaration. These answers are stored in the site owner’s Wix forms dashboard. A percentage and provisional tier are calculated from the draft v1 rubric on your device after successful submission. The result is not published automatically and does not replace evidence review. Do not include patient information.
Applicant accounts, evidence and profile drafts
Clinician accounts link an application to a signed-in member. Profile drafts are readable by that member and authorised administrators; the review queue and evidence-file records are administrator-only. Evidence PDFs are uploaded as private files. A signed-in applicant can request a short-lived download link only for evidence recorded against their own account.
Referee reports contain professional contact details, the referee’s relationship to the applicant and observations about work experience. Obtain the referee’s permission to share the report for application review. Reports and referee details are private supporting evidence and are not published in the directory.
Photographs and company logos are public media assets when uploaded; only upload images intended for publication and for which you hold permission. The draft profile itself is not listed publicly until the moderator publishes an approved copy. Public contact details and approved profile information are then visible in the directory. Supporting evidence, registration details and private account information are not copied into the public profile.
When an applicant submits for review, a general network enquiry is also created to request the existing moderator email notification. That notification contains the application reference and review links, not the private evidence files.
Before live patient use
Care Loop generates a PDF on your device. The optional email file includes the named coordinator as recipient and the PDF as an attachment; downloading it does not send it. Review and send it through an approved communication channel. The care coordinator distributes it to the authorised team, and receipt, action tracking and retention happen in your existing care-record system. Confirm appropriate authority and consent before sharing. Urgent concerns need direct communication. The site does not provide a patient database, audit trail, notification service or clinical record.
Privacy arrangements to finalise
The operating entity, privacy contact, retention schedule, access and correction process, service-provider arrangements and complete privacy policy need to be confirmed before the network begins operating. This pilot information page is not a completed clinical-service privacy policy.
