Less repeating.
More connection.
The MND situation is the starting point for Neuro Clinician Network: finding relevant experience and helping independent providers keep a shared picture of care.
Several clinicians are involved. Each sees a different part of the picture. The family is left carrying the information between them.
A clinician notices a change during a home visit. It is recorded in their own notes, but the rest of the team may not know. The person or family must explain it again.
This is the communication gap the network is designed to address.
Find relevant experience
A referrer needs an MND-experienced clinician. The directory makes condition-specific experience, clinical settings and service fit visible.
Understand the care team
The person and their authorised team need clarity about who is involved, their role and how they can be contacted.
Share changes after appointments
Each appointment produces a concise update: functional status, concerns, plan and actions. A no-change update still keeps the communication loop intact.
Acknowledge and act
A request has a named owner. Acknowledgement and completion help the team see whether follow-through occurred.
The person stays
at the centre
Care Loop updates reflect the person's goals and communication preferences. The care coordinator distributes each report only to the authorised team.
Patients and families need a clear explanation of who receives each update and appropriate consent for sharing. Completed forms are downloaded and managed through existing care-record systems, not stored on this site.
Open Care Loop checklists ↗Questions about this first version
Can I book an MND clinician here today?
Use a clinician’s published contact details to discuss availability, clinical fit and funding. Test profiles are clearly marked. This website does not receive clinical referrals or book appointments.
Is Care Loop a clinical service?
Care Loop is a checklist and downloadable appointment-update tool for PT, SP, OT, dietetics and registered nursing (RN). The clinician sends the report to the care coordinator for distribution. It is not a monitored patient record or an urgent messaging channel.
Does the network replace existing providers?
The proposed model connects independent providers. Each clinician continues to hold their own clinical records and responsibility for their care.
How will clinician experience be checked?
The proposed criteria assess years, settings, intensity and recency of neurological practice, with condition-specific experience made visible. Final scoring and verification arrangements are still being developed.
